Step 1: Prepare and submit the dossier.
-Employees: Submit the dossier to their office.
-People participating in voluntary social insurance, households participating in health insurance, participants whose health insurance premiums are paid partially by the state budget: Submit the dossier to authorized collectors or social insurance authorities.
– Participants whose health insurance premiums are paid totally by the state budget: Submit the dossier to the Commune People’s Committee or social insurance authorities, sanatoriums taking care of people with meritorious services to the revolution; vocational education institutions affiliated to the branch of labor, war invalids and social affairs.
– Participants whose health insurance premiums are paid totally by social insurance organizations: Submit the dossier to the Commune People’s Committee or social insurance authorities.
– Students whose health insurance premiums are paid through their schools shall submit their dossier to their schools.
NOTES: After finishing the declaration, participants shall sign and write their fullname. If declaration to change the personal information (family names, names, middle names; date of birth; gender) prescribed in social insurance books, health insurance card, employees must request for confirmation of their office where they are working. People who preserve their payment period of social insurance need not do this confirmation.
Step 2: Receive, declare, submit the dossier.
- Employers:
+ Receive the dossier.
+ Make a list of information according to the specimen D01-TS (if any).
+ Declare the dossier for reissuance and adjustments to contents (using the specimen TK1-TS) in case of adjustments to his/her family name, middle name, date of birth and gender specified in his/her social insurance book.
+ Submit the dossier to social insurance authorities.
- Schools, the Commune People’s Committee, sanatoriums taking care of people with meritorious services to the revolution; vocational education institutions affiliated to the branch of labor, war invalids and social affairs:
+ Receive the dossier;
+ Declare the dossier;
+ Submit the dossier to to social insurance authorities.
